For those countries whose health systems are facing significant decline, or significant new threats, such as a high level of HIV/AIDS related illness, choices need to be made about:

• how national resources are to be shared;
• the weighting given to health;
• where and how to spend public resources within public health systems;
• how to motivate and direct private health spending;
• how household contributions will be balanced against state contributions.
